My MH Chart Eligibility

Am I Eligible for a Patient Portal (My MH Chart) Account?

In order to be eligible for a MY MH CHART patient portal account you simply must have received healthcare services at Methodist Health Henderson or Methodist Health Union County. If you meet this eligibility requirement, listed below is the information you will need to be able to set up your MY MH CHART account:

  • First and Last Name
  • Date of Birth
  • Medical Record Number (if you do not know this number, contact Medical Records)
  • Valid Email Address
  • Access to a computer
  • Internet Access from your computer
