Medical Records

Health Information Management (HIM), or as it is commonly known "Medical Records" is the department in the hospital that organizes, analyzes, and manages each patient's health information to insure its quality, accuracy, accessibility and security. This department ensures appropriate disclosure of information in both paper and electronic systems.

Patients can request an electronic copy of their medical record be sent to another provider from within the MY MH CHART portal. However, this can only be accomplished if the other provider has a secure site for receiving the electronic file.

Patients desiring to secure a paper copy of their medical record from Methodist Health must submit a completed and signed Release of Information form to one of the following, depending upon which facility provided the healthcare:

For convenience, a copy of the Release of Information Form is accessible on the Patient Portal Page under FORMS. Although you may have been born at Methodist Health, the State of Kentucky Office of Vital Statistics is the official repository for all Kentucky birth records. Methodist Health is not legally able to issue a copy of your Birth Certificate. In order to obtain a copy of your Birth Certificate you may do one of the following:

• Go to http://chfs.ky.gov/dph/vital/birthcert.htm and request a copy electronically
• Fax a request to 866-283-7477 (this is a toll free number)
• Call 800-241-8322 toll free
